When booking a face to face service, you are agreeing that you do not have any Covid -19 Symptoms and that you are not feeling unwell or visited / living in any Covid-19 Declared Hot Spot Areas.​For the purpose of Contact Tracing as outlined in the Queensland Health Government Website at https://www.health.qld.gov.au you understand your Name, Address, Phone number and times in and out of attending face to face servicing may be accessed for the Purpose of Contact Tracing.
